A rash of bizarre murders in New York City seems to point to a group of grotesquely deformed vagrants living in the sewers. A courageous policeman, a photo journalist and his girlfriend, and a nutty bum, who seems to know a lot about the creatures, band together to try and determine what the creatures are and how to stop them.

Za mnogobrojne beskućnike grada New Yorka počinju crni dani, jer tijekom kratkog perioda počinju nestajati svi oni kojima je nastamba blizu kanlizacija. Policija tapka u mraku i nikako ne nalazi riješenje problemu.
A.J., jedan od prijatelja beskućnika, vodi istragu na svoju ruku i dolazi do jako uznemirujućih spoznaja, a u istrazi mu pomaže i kapetan Bosch, čija je žena također nestala.
Njih dvoje doznaju da u kanalizacijama žive mutanti-ljudožderi koji su nastali zbog radioaktivnog otpada kojeg je jedan vladin agent godinama spremao u kanalizaciju ispod grada. Mutanata je mnogo i žrtve još padaju … Ali policija ima plan za njihovo uništenje. No, koliko će stvarno biti učinkoviti i hoće li uspijeti potamaniti sve krvožedne stvorove, morat ćete otkriti u filmu.
